| Please note that workers' compensation information must be submitted with all applications. Click here to access the affidavit that must be completed and submitted along with the certificate of insurance showing a current policy with a policy number and policy expiration date. If you are a sole proprietor, and workers' comp is not required,you will check that box, sign, and submit this affidavit. It is also a requirement that you provide a copy of professional license(s) and HIC registration (if you are a registered contractor) with each and every application. You will need to make copies of these licenses and present with your permit application; we do not offer copying services for licenses. Thank you! |
